Job Description

We are seeking a Nurse Practitioner (NP) to join our team for an in-person role located in London , ON. In this position, you will be responsible for providing primary care services to senior patients. You will address medical concerns, order diagnostic tests, prescribe medications, perform routine check-ups, and work collaboratively with our interprofessional healthcare team to ensure the highest standard of care for seniors.

Responsibilities

  • Primary / Urgent Care for Seniors : Serve as the main healthcare provider for patients, offering timely assessments, diagnoses, and treatments for a wide range of medical concerns. Provide on-site services at the retirement residence in Hamilton and offer virtual follow-ups as needed.
  • Patient Consultations and Assessments : Conduct thorough consultations and assessments to determine the best course of treatment tailored to each client's individual needs.
  • Health Status Documentation : Assess and document patient health status and needs accurately.
  • Care Plan Development : Develop and implement individualized care plans based on assessment findings.
  • Medication and Treatment Administration : Administer medications and treatments as ordered by the physician.
  • Patient and Family Education : Educate patients and their families on disease prevention, management, and overall wellness.
  • Preventative & Chronic Disease Management : Provide guidance on preventative health measures, including immunizations, screenings, and lifestyle interventions. Offer proactive management of chronic conditions, coordinating care with other healthcare professionals when necessary.
  • Record Maintenance : Maintain accurate patient records in electronic medical records (EMRs) and ensure compliance with HIPAA regulations.
  • Team Collaboration : Collaborate with other healthcare professionals, including physicians and nurse practitioners, to ensure continuity of care and optimal treatment outcomes.
  • Quality Improvement : Participate in quality improvement initiatives and stay up-to-date on best practices and clinical guidelines in aesthetics and nutrition.

Requirements

  • 2+ years of clinical nursing experience, preferably with a focus on primary care, urgent care, or geriatrics.
  • Master of Nursing or equivalent, with a clinical focus.
  • Completion of a Nurse Practitioner program (MN or post-master certificate).
  • Registration with the College of Nurses of Ontario in the RN (Extended Class).
  • Strong communication and interpersonal skills, particularly with older adults, families, and multidisciplinary teams.
  • Experience or strong interest in geriatric care and preventative medicine is preferred.
  • Strong clinical assessment and consultation skills.
  • Excellent communication and interpersonal skills for patient education and team collaboration.
