Job Description

We are seeking a Nurse Practitioner (NP) to join our team for an in-person role located in Hamilton, ON. In this position, you will be responsible for providing primary care services to senior patients. You will address medical concerns, order diagnostic tests, prescribe medications, perform routine check-ups, and work collaboratively with our interprofessional healthcare team to ensure the highest standard of care for seniors.

Responsibilities

  • Primary/Urgent Care for Seniors: Serve as the main healthcare provider for patients, offering timely assessments, diagnoses, and treatments for a wide range of medical concerns. Provide on-site services at the retirement residence in Hamilton and offer virtual follow-ups as needed.
  • Patient Consultations and Assessments: Conduct thorough consultations and assessments to determine the best course of treatment tailored to each client's individual needs.
  • Health Status Documentation: Assess and document patient health status and needs accurately.
  • Care Plan Development: Develop and implement individualized care plans based on assessment findings.
  • Medication and Treatment Administration: Administer medications and treatments as ordered by the physician.
  • Patient and Family Education: Educate patients and their families on disease prevention, management, and overall wellness.
  • Preventative & Chronic Disease Management: Provide guidance on preventative health measures, including immunizations, screenings, and lifestyle interventions. Offer proactive management of chronic conditions, coordinating care with other healthcare professionals when necessary.
  • Record Maintenance: Maintain accurate patient records in electronic medical records (EMRs) and ensure compliance with HIPAA regulations.
  • Team Collaboration: Collaborate with other healthcare professionals, including physicians and nurse practitioners, to ensure continuity of care and optimal treatment outcomes.
  • Quality Improvement: Participate in quality improvement initiatives and stay up-to-date on best practices and clinical guidelines in aesthetics and nutrition.

Requirements

  • 2+ years of clinical nursing experience, preferably with a focus on primary care, urgent care, or geriatrics.
  • Master of Nursing or equivalent, with a clinical focus.
  • Completion of a Nurse Practitioner program (MN or post-master certificate).
  • Registration with the College of Nurses of Ontario in the RN (Extended Class).
  • Strong communication and interpersonal skills, particularly with older adults, families, and multidisciplinary teams.
  • Experience or strong interest in geriatric care and preventative medicine is preferred.
  • Strong clinical assessment and consultation skills.
  • Excellent communication and interpersonal skills for patient education and team collaboration.
