Job summary

Calgary West Central Primary Care Network (CWC PCN) is seeking a Primary Care Registered Nurse (PCRN) for a temporary maternity leave contract in the Calgary Zone. The role involves holistic patient assessment, health promotion, and collaboration with an interprofessional team to improve patient outcomes.

The successful candidate will have a Bachelor’s degree in nursing, active CRNA registration, and experience in chronic disease management and mental health.

Qualifications

• Education: a bachelor’s degree in nursing is required.
• Active CRNA registration in good standing is required.
• Experience in broad clinical practice including mental health and chronic disease management is required.
• Certification in CDE or willingness to obtain is an asset.

Responsibilities

• Counsel and guide patients on symptom management, health maintenance, and rehabilitation strategies, and involve them in decision-making and self-management.
• Provide and coordinate screening for health issues such as diabetes, hypertension, cholesterol, cardiovascular health, mental health, and cognitive function.
• Work collaboratively with the interprofessional team to identify, plan, implement, and measure patient-centred health improvement goals.
• Implement and facilitate groups for health promotion.
• Complete comprehensive and person-centred assessments, develop care plans synthesizing health assessment information, and use critical thinking and clinical reasoning to identify health concerns and interventions.
• Use HealthChange Methodology to provide person-centred care and promote health literacy, shared decision-making, behaviour change, and self-management.
• Assist in setting up health promotion programs and patient self-management programs.
• Direct patients to the care they need from the appropriate health professional or program.
• Collaborate with other teams to identify patient groups and target goals to improve patient care.
• Participate in identifying new service requirements and address deficiencies for patient benefit.
• Offer training and support for other patient care team members and assist in their professional development.
