Job summary

Bayshore Medical Personnel seeks a full-time RN Primary Care (PCN) in Saint-Jean-sur-Richelieu, QC to deliver primary health care within a collaborative family practice.

The role includes patient assessment, screening, lifestyle coaching, education, and chronic disease management, aiming to improve health outcomes and access to care. You will work with a multidisciplinary team on walk-in and scheduled visits for acute, chronic, mental, and occupational health.

Qualifications

• Minimum of one year of full-time equivalent experience in family practice, ambulatory care or a health care setting.
• Bachelor of Science in Nursing (BScN) from a recognized university
• Registered Nurse license in the province of practice
• BLS for Healthcare Provider (CPR/AED Level C) or CPR/AED Level C

Responsibilities

• Provide primary health care services to clients in a primary care setting.
• Perform patient assessment, screening and health-maintenance education.
• Manage chronic diseases and coordinate care with a multidisciplinary team.
• Schedule and conduct walk-in and planned visits for acute and chronic needs.

Bayshore Medical Personnel is seeking a full-time RN Primary Care (PCN) in Saint-Jean-sur-Richelieu, QC to provide service to our client in a primary healthcare setting. $1000 Signing Bonus

PCN's are responsible for the provision of primary health care services to members, and to provide patient assessment, screening, healthy lifestyle support, education, and chronic disease management with a goal of improving health outcomes and facilitating access to health services.

They function as a member of a multi-disciplinary health care team to develop and implement care plans and provide patient follow-up. Their work includes walk-in and scheduled appointments with acute or chronic physical, mental, and occupational health needs.

Qualifications

Successful candidates must possess a minimum of one (1) year of full-time equivalent experience in family practice, ambulatory care environment or in a health care setting.

Candidates Must Also Possess

• Baccalaureate Degree in Nursing from a recognized university
• Registered Nurse license from the provincial or territorial professional college or association in the province of practice
• Certificate of Basic Life Support (BLS) for Healthcare Provider or CPR/AED Level C

Additional Requirements

• Current N95 mask fit certificate CSA Standards Z94.4 (every 2 yrs)
• Workplace Hazardous Material Information System (WHMIS) Certification (every two years)
