Practice Nurse required for a busy surgery in Milton Keynes, excellent package available £££'s.

We have an exciting opportunity for a part-time Practice Nurse to join a busy GP surgery in Milton Keynes, working with a friendly team and reporting to the Group Lead Nurse. The Practice Nurse Coordinator is responsible for overseeing the Practice Nursing Hub; including the planning and co-ordination of the Practice Nursing Hub rotas.

Duties will include:

  1. Look for opportunities to improve patient care whilst maximising direct & Local enhanced services & QOF.
  2. Implement and evaluate individual treatment plans for patients with Long-term conditions.
  3. Identify and manage appropriately patients at risk of developing a long-term condition.
  4. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including the initiation of effective emergency care.
  5. Support patients to adopt health promotion strategies that encourage good health, deliver opportunistic health promotion at all times and support the principles of self-care.
  6. Provide information and advice on prescribed or over-the-counter medication on medication regimes, side effects and interactions.
  7. Support and advise women requesting information on contraception and sexual health.
  8. Support and manage health needs of women presenting for cervical cytology consultations, obtaining cytology samples in accordance with the cervical screening programme and competencies.
  9. Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health.
  10. Implement and participate in vaccination and immunisation programmes for both adults and children.
  11. Advise, support and if required, administer vaccinations for patients travelling abroad.

Essential for the role:

  1. Be a certified Registered Nurse.
  2. Have a current NMC registration.
  3. Be a specialist practitioner qualified in Adult Nursing.
  4. Be a Non-Medical Prescriber or willing to work towards.
  5. Evidence of ongoing CPD.
  6. Have experience in respiratory conditions.
  7. Have clinical skills (cervical cytology, immunization, vaccination, basic wound care, assisting the seniors in minor surgery sessions, ear care).
  8. Desirably, chronic disease management, CHD, diabetes, asthma and COPD, complex wound care, spirometry, LARC, pessary insertion/removal.
  9. Experienced with System one.
