A leading healthcare authority is seeking a Practice Consultant for Home Health to improve health outcomes for First Nations individuals. This role is responsible for providing clinical consultation and education support for healthcare teams, enhancing service delivery and program planning in home care.

Salary : $102,490 – $125,736 per year (pay range explained below)

Contract : Permanent

Hours : 37.5 hours, Monday – Friday (core hours 9am-3pm)

Location : Sinclair, Downtown Vancouver – Hybrid available. Please note, this role will be relocating to our new office building in North Vancouver on Tsleil-Waututh Nation (TWN) land, south of Mount Seymour Parkway, spring 2027.

About The Role

As Practice Consultant – Home Health, you will – provide professional practice, education support, and clinical practice consultation support for regional community health practice consultants and health care providers who provide home care and home health services to First Nations individuals across the life span. You will contribute to program planning, guideline and resource development to support knowledge / practice in areas of home care, chronic conditions, and serious illness. You will facilitate planning and enhancement supports for Home and Community Care programming that supports the health and wellbeing of all individuals and other clients receiving these services.

About You

To be successful in this role you will have / be –

  • Bachelor’s degree in nursing
  • Master’s degree considered an asset
  • Additional education in home care or long-term care is an asset
  • Current practicing registration with the BC College of Nurses and Midwives
  • Five (5) years of recent clinical experience in a home care setting and / or dual-role (community health nursing and home care), preferably in a First Nations community.
  • Experience in planning and delivering educational programs for nurses and health care providers
  • Experience working in or with First Nations communities delivering health or social services
