Job description

Responsible to: Director of Programs (D of P)

Areas of Responsibility: Post-Secondary Program

Post-Secondary Program: 40 hours per week

Responsibilities
  1. Oversee participant recruitment and intake
  2. Attend Inner Hope and community events to build relationships with prospective participants and their families
  3. Partner with Boundless Mentors and Boundless Program Coordinator to transition program participants from Boundless into the Post-Secondary Program
  4. Follow up on referrals from current participants and Inner Hope contacts
  5. Facilitate program intake (application form, participation agreement, etc.)
Case Management
  1. Manage relationships with 12-15 program participants as a mentor, confidant, and role model
  2. Complete monthly check-ins with program participants
  3. Facilitate each participant’s development of an Education, Employment and/or Life Skills Plan (every 6 months), using the Medicine Wheel to develop goals that support a balanced way of living focused on the four aspects of physical, emotional, mental and spiritual well-being
  4. Support participants in achieving goals, in partnership with community organizations, tutors, coaches, family members, and/or other Inner Hope personnel
  5. Hold participants accountable to program expectations
  6. Coordinate crisis intervention when necessary, with support from other program team members
  7. Support family members of program participants and other core participants who fall under the “Support” program area as time allows (e.g. moving, meals, rides, crisis intervention)
Volunteer Support
  1. Collaborate with Volunteer Coordinator to facilitate recruitment of program volunteers when needed (tutors, employment coaches, event support, etc.)
  2. Attend semi-annual Volunteer Workshop with all program volunteers
  3. Meet individually with the program’s core volunteers at minimum once a year to hear their feedback and provide support
Coordinate Events for Participants
  1. Host workshops and/or facilitate drop-in Young Adult Nights to support young adults in achieving goals with assistance from volunteers (set-up, transportation, food, supervision and clean-up as needed, activities reflect the current needs of the group: i.e. community building or goal focus)
  2. Plan, promote, and host quarterly social gatherings (i.e. winter tubing or skating, pumpkin patch, summer pow-wows, etc.). Additional events can be planned as time and budget allows.
  3. Partner with Cultural Mentors to ensure program elements are culturally inclusive and offer contextualized opportunities to explore faith
  4. Host larger events throughout the year with volunteer support (annual camping trip, Christmas)
Network with Community Agencies
  1. Maintain working knowledge of local resources available to vulnerable youth (e.g. counselling, housing services, subsidy, treatment centres, etc.) including those related to Indigenous participants and those who have had involvement or been in MCFD/VACFSS care (i.e. band funding, tuition waivers, SAGE, FNHA, Jordan’s principle, etc.)
  2. Build external relationships to increase referrals to Inner Hope
  3. Connect participants with community resources and advocate for their needs
  4. Champion the wraparound services of Inner Hope in all external relationships
Maintain Records
  1. Collaborate with Director of Programs to set annual program targets
  2. Track program outcomes, report monthly and annually to Director of Programs
  3. Maintain participant records (intake forms, medical information, goals, applications, etc.)
  4. Complete reports on Post-Secondary Program for funders
  5. Manage program budget
Develop and Revise Project Materials as Needed
  1. Intake forms, resources for participants, etc.
Oversee Maintenance of Inner Hope Vehicles
  1. Renew annual BCAA membership
  2. Ensure regular scheduled maintenance on all IHYM vehicles is done (i.e. oil changes, tire changes/rotation, etc.)
  3. Advise DOP of any vehicle issues, create plan and schedule repairs as needed within budget
Partner with IHYM Staff
  1. Attend weekly staff meetings and one-on-one meetings with supervisor
  2. Collaborate with program team on cross-program events and/or participant discipleship opportunities
  3. Participate in employer-funded professional development activities on and off site
  4. Be an ambassador for IHYM:
    1. Seek to expand its donor/volunteer network
    2. Assist where needed with major fundraising initiatives
    3. Lead a team in the annual Ride for Refuge, recruiting Post-Secondary participants to ride and fundraise

All other duties as assigned by supervisor
