Job summary

Qualifications

Responsibilities

Physiotherapy Assistant - Bonnechere Manor

Overview

Reporting to the Client Outreach Programs Supervisor and under the directions of a licensed qualified physiotherapist, the Physiotherapy Assistant delivers goal oriented therapeutic programs to long-term care residents, enabling him or her to maintain or improve functional and cognitive capacities in all aspects of daily living, to the extent of his or her abilities. Performs duties in accordance with Bonnechere Manor’s Mission, Vision and Values, Competencies and the Long-Term Care Homes Act and Regulations.

This is a part-time contract up to 18 months.

Responsibilities

• Acknowledging individual’s strengths and limitations identifies resident’s needs and wishes. In conjunction with the interdisciplinary team and through effective communication, establishes specific therapeutic goals and interventions which focus on prevention, maintenance and progression of person-centred wellbeing.Updates the resident plan of care to reflect this as needed.
• Utilizing a consistent, person-centred approach to meet resident needs and demonstrates initiative to:
• Obtain appropriate in house short term seating if resident has a sudden change of condition or requires alternative solutions for seating,
• Responsible for tagging all mobility equipment, both resident and facility owned, and logging inventory on resident file and within rehabilitation department,
• Coordinates biannual preventative maintenance program of all wheelchairs and walkers, both resident and facility owned. Identifies all mobility devices and rehabilitation equipment requiring repair or adjustments and initiates corrective action after supervisor or resident approval,
• Maintains resident over bed transfer logo signs and updates as required,
• Assists with the resident dining process by serving meals/feeding,
• Follows the Physiotherapist’s treatment plans for the residents and documents accordingly as per RAI MDS requirements,
• Ensures referrals for Physiotherapy, Occupational Therapy, or Speech Language assessment are coordinated in a timely and efficient manner,
• Coordinates appointments and accompanies therapists during resident assessment and treatment, implementing prescribed equipment/programs as instructed by therapist, and
• Participates in annual and admission care conferences as required by providing accurate updates.
• Assists in identifying educational needs of rehabilitation program and actively participates in workshops, educational programs. Assists with new and ongoing education and orientation to nursing staff relative to safe client handling, back care, and proper use of mechanical lifting equipment.
• Establishes and maintains effective communication and liaison between therapist, resident, families, organization and documents all relevant information with respect to resident goals, progress, and outcomes in resident medical file. Assists to support the interdisciplinary team by documenting in the resident file, communication books and on shift report as needed. Maintains ongoing communication with residents and service providers to ensure equipment is appropriate for residents.
• Maintains competency through self-directed professional development by participating in professional group activities such as workshops, in-services to ensure skills and abilities are maintained and enhanced and through applying skills to day-to-day role/functions.
• Participates in Continuous Quality Improvement (CQI) initiatives by; participating in the review and development of resident and departmental goals, objectives, policies and procedures Physiotherapistand by being an active participant in the budgetary process of identifying rehabilitation equipment and resource materials required by residentsand/or facility. Maintaining inventory of rehabilitation equipment/resource material are also assigned duties in the CQI process.
