Through a new, innovative partnership with Nova Scotia Health (NSH), we are opening the first transitional health facility in our province, in West Bedford. Shannex is the operating partner for the transitional health facility on behalf of Nova Scotia Health. You will have an opportunity to work in a facility with a purpose and design dedicated to providing care to NSH patients in the central zone, who no longer require hospital care, but need services before they can safely return home/community.

We are searching for a Physiotherapist to join our Transitional Health Facility located in West Bedford, Nova Scotia.

The transitional health model supports NSH patients/families who require additional time, services, and support to transition back to home/community, by providing an alternate space with intentional programming, building design, and resource allocation to increase the probability of returning to their previous living situation.

Patient specific care focuses on maintaining and/or improving functional abilities and health status in a positive environment where the Physiotherapist encourages adherence to treatment plans and preventative care guiding patients/families with their knowledge and expertise.

The Physiotherapist is integral in supporting the goal of returning to home/community by being engaged in interprofessional collaboration, offering evidence-based clinical expertise to inform care plans that fit with patient/family goals, using effective communication strategies, shared decision-making, and mutual support.

The Physiotherapist supports a commitment to ongoing quality improvement, continuous exploration and implementation of new approaches, learning, teaching, and mentoring.

About the Opportunity

  • Assesses, diagnoses, and treats patients in a tertiary care setting who present with actual or potential impairment, pain, functional limitations or disabilities.
  • Encouraging, motivating, and assisting patients with hygiene and daily living activities, such as: bathing, dressing, oral care, skin care, meals, and mobilizing, according to the patient’s care plan.
  • Plans intervention strategies by promoting active involvement of the patient, family, or caregiver in establishing physiotherapy goals.
  • Participates in interdisciplinary rounds, team meetings and family meetings regarding patient care.
  • Develops individualized treatment plans based on clinical records, observation, or referral.
  • Maintains an inventory of physiotherapy equipment and supplies.
  • Ensures patient safety by following and applying all standard operational procedures.

About You

  • A master’s degree in Physiotherapy.
  • Current registration with the provincial College of Physiotherapists.
  • Minimum of two years clinical experience.
  • Knowledge of the principles, methods, techniques, materials, equipment, and procedures used in physiotherapy.
  • Demonstrated ability to exhibit high standards of ethical conduct, honesty and integrity.
  • Ability to communicate effectively and efficiently, both verbally and in writing.
  • Knowledge of skeletal, muscular and neural anatomy and physiology as it applies to physiotherapy.
