Job description

Position: Physiotherapist

Location: Goderich, ON

Southbridge Care Homes, in partnership with our clinic managed by Back in Motion, is seeking a Physio Therapist, who is ADP authorized, to work out of our new long term care home in Goderich ON. They will be expected to deliver support and expertise to both the residents of the home as well as external clients visiting the clinic.

Southbridge Care Homes owns and operates 26 Long-Term Care Homes and 9 Retirement Homes throughout many urban and rural areas of Ontario.

The successful candidate will demonstrate flexibility, adaptability, and passion for senior care by delivering high-quality leadership support to our clinic.

Resident Focus:

  • Conduct Physiotherapy Assessments as per referrals
  • Provide assessments and access funding under the Assistive Devices Program (ADP) to meet equipment needs, in compliance with all ADP required activities and documentation, as well as funding guidelines. ADP authorizer with power wheelchair qualifications preferred.
  • Create and oversee Resident- and Family-centered Care Plans; establish personalized treatment goals in collaboration with the Resident/Family.
  • Implement, monitor, and modify treatment to achieve Resident/Family goals in the areas of seating and positioning, activities of daily living, equipment use and safety, and strategies to improve function.
  • Participate in interdisciplinary care planning and Care Conferences, as well as Committees as required.
  • Prepare documentation and reports according to College and organizational standards.
  • Participate in continuous quality improvement program, including measurable outcomes, satisfaction feedback, and quality improvement planning.

Community and additional responsibilities:

  • Conducting assessments and establishing and implementing evidence-based treatment plans.
  • Performing functional assessments to determine baseline function and to identify return to work barriers.
  • Developing a treatment plan comprised of program goals program duration, return to work recommendations and anticipated outcome.
  • Adhering to standards regarding the timeliness of reports.

Requirements:

  • Registrant in good standing with the College of Physiotherapists of Ontario.
  • Certified Assistive Devices Program (ADP) Authorizer required OR close to completion.
  • Knowledgeable and experienced in evidence-based musculoskeletal and neurological practice with a focus on functional outcomes.
  • Possess superior communication and clinical reasoning skills.
  • Caring and compassionate in all interactions with Residents and Families.
  • Business Development experience.
  • Completed a satisfactory Police Vulnerable Sector Check (VSC) upon hire.
  • Up to date on required vaccinations, including influenza vaccine, MMR, DTaP, varicella, and TB skin test (Only for LTC work).
