Job summary

Carefor is seeking a Physiotherapist to work in community settings across Eastern Ontario, empowering clients to achieve rehabilitation goals through personalized treatment and education. You will provide hands-on care, build independence, and collaborate with case managers and a multidisciplinary team.

You will treat clients with acute and chronic conditions, deliver services at full scope of practice, participate in ongoing training, and contribute to a supportive not-for-profit organization

Qualifications

  • Registration in good standing with the College of Physiotherapists of Ontario.
  • Strong clinical expertise focusing on rehabilitation and functional recovery.
  • Excellent communication and teaching skills to inspire clients and families.
  • Strong critical thinking, problem-solving, and sound decision-making.
  • Promote autonomy for clients and families to live independently.
  • Work independently and within a dynamic care team.
  • Valid G2 or higher driver's license and reliable vehicle.
  • 2–5 years of clinical practice experience preferred.
  • ADP Authorizer status an asset.
  • Community health/home care experience is an asset.
  • Bilingual in French/English considered an asset.

Responsibilities

  • Restore movement through exercise, strength training, mobility retraining, and balance programs
  • Support rehab needs including musculoskeletal, neurological, pulmonary, and cardiovascular care
  • Develop personalized care plans reflecting client goals
  • Measure and track progress with evidence-based tools
  • Educate and empower clients and their families for self-care and long-term health
  • Prescribe and train in the use of mobility aids, adaptive devices, transfer aids, and rehab equipment
  • Collaborate with case managers and interdisciplinary team members to deliver client-centered care
  • Refer clients to appropriate community resources and support

Qualifications
