Job summary

St. Joseph's Home Care in Hamilton, Ontario is seeking a Physiotherapist to assess and plan personalized rehabilitation programs for clients transitioning from hospital to home. You will work with families and healthcare teams to ensure safe, meaningful care and support mobility in home settings.

Qualifications

  • Bachelor’s or Master’s Degree in Physiotherapy (Master’s preferred).
  • Registration with the College of Physiotherapists of Ontario (CPO)
  • Current BCLS/HCP certification
  • 1–3 years of clinical experience (required)
  • Experience with complex medical conditions and mobility support
  • Home and community care experience (preferred)
  • Familiarity with mobile work tools and EMR systems.

Responsibilities

  • Conduct client assessments and develop personalized treatment plans
  • Provide education on therapy goals, exercises, and adaptive equipment
  • Collaborate with healthcare teams for care coordination and discharge planning
  • Supervise support staff and mentor students
