The Physiotherapist is a key member of the community-based Early Supported Stroke Discharge Team, reporting to the Program Manager – ABI. The role involves working within an inter-professional team to deliver interventions aimed at supporting recently discharged, community-dwelling stroke patients requiring ongoing rehabilitation to remain in their communities.
The Physiotherapist assesses, diagnoses, and treats patients in community and tertiary care settings, addressing impairments, pain, functional limitations, or disabilities. Responsibilities include medical history review, standardized testing, developing holistic treatment programs, and educating patients and families. The role also involves supervising students and assistants, collaborating with colleagues, and participating in team evaluations. Weekend work and coverage in other rehabilitation areas may be required.
