Job Summary:

Ambulatory Services has an opportunity for a Physiotherapist to join the Team. This position will support Ambulatory Services Day Hospitals and Outreach Teams. This position will work creatively in a variety of community environments and programs, such as outpatient, day hospitals, outreach teams, community housing, primary care clinics, etc., including using online platforms to provide virtual access to care.

Responsibilities include but are not limited to:

  • Conducts clinical assessments, care planning, treatment interventions, and discharge planning for a mix of complex geriatric, neurological, and musculoskeletal populations.
  • Participates actively in the teaching and mentoring of physiotherapy and physiotherapy assistant students.
  • Assigns the patient plan of care, and provides direction, supervision & monitoring of physiotherapy assistants.
  • In collaboration with the inter-professional team, implements a client-focused care plan and coordinates the discharge planning process for the patients.
  • Provides support/consultation to other disciplines on the team.
  • Comfort assigning to OTA/PTA when appropriate.
  • Supports a systematic, evidence-based approach to client care, program development and evaluation, and professional practice, in close collaboration with inter-professional team members, volunteers, and the Physiotherapy clinical team.

Qualifications include but are not limited to:

  • Licensure in good standing with the College of Physiotherapists of Ontario.
  • Membership or eligibility for membership with the Canadian Physiotherapy Association is an asset.
  • Experience with a geriatric population with multiple medical conditions and/or cognitive impairment preferred.
  • Confirmed status as ADP authorizer required.
  • Expertise in clinical assessments of elderly clients.
  • Ability to lead groups and to plan/execute knowledge translation/adult learning sessions with clients, caregivers, family members, and inter-professional team members.
  • Experience in providing clinical instruction and teaching to physiotherapy students is an asset.
  • Previous experience and/or interest in developing evidence-based programs is an asset.
  • Excellent interpersonal, communication, time management, and problem-solving skills.
  • Access to transportation (e.g., current driver’s license and car) required.
  • Proficient computer skills – Meditech, MDS, Microsoft Word, PowerPoint, and Excel required.
