Job Description

What we are looking for:

As a member of our interprofessional team, you will be responsible for providing assessment, consultation, and education to patients, families, and the interprofessional team within the adult patient population at Oak Valley Health - Markham site, the Reactivation Care Centre.

You will utilize a goal-oriented, client-centered approach to care and participate in program development and quality improvement activities.

Physical demands include frequent moderate to heavy exertion: lifting over 60 lbs., manual patient handling including chest care, repositioning, transferring, lifting, and ambulation activities.

What you bring to the role:

  • Current registration with the College of Physiotherapists of Ontario
  • At least 2 years of experience in an acute and/or subacute care setting
  • BLS Certification
  • Experience with surgical, rehab, and emergency patient populations
  • Ability to work effectively in a fast-paced environment
  • Strong critical thinking skills
  • Excellent discharge planning skills and knowledge of community resources
  • Excellent communication and interpersonal skills
  • Ability to collaborate within an interprofessional team including support staff
  • Good computer skills; experience with electronic documentation preferred
  • Evidence of ongoing professional development
  • Good attendance and performance records
  • Commitment to workplace safety and compliance with policies and legislation

Work Schedule:

Monday to Friday, 8- or 10-hour shifts with rotating weekends. Schedules may change due to operational needs.

Compensation:

OPCOM12, Range: $41.36 - $57.27
