As a Physiotherapist (Status: Permanent, Full-Time; Location: Sault Ste. Marie; Start date: Immediately), you will provide physiotherapy services to Ontario Health atHome patients to reduce, eliminate, or prevent physical disabilities/impairments and improve the patient’s functional independence and safety in their environment.

Whether you work in our office, in the community, or a healthcare facility, you will play a lead role in providing connected, accessible, patient-centered care, supported by our collaborative team of over 9,000 regulated health care and other professionals.

Your mission as a valued team member will be to help our patients be healthier at home, while benefiting from our support for professional growth, personal wellness, and work-life balance.

What will you do?

  • Respond to patient referrals from Care Coordinators to administer physiotherapy services.
  • Assess patients' physical abilities to determine the impact of their condition on their functionality.
  • Develop and implement treatment programs based on assessment results.
  • Monitor patient progress and adjust treatment plans as needed to meet therapy goals.
  • Assign physiotherapy tasks to rehabilitation assistants based on patient needs and supervise according to professional standards.
  • Educate and train patients, families, caregivers, and SDMs on treatment plans.
  • Connect patients with other services or organizations if needed.
  • Respect patients' values and preferences while balancing their needs with available resources.
  • Document assessment results, goals, treatment plans, and interventions accurately.

What must you have?

  • Registered Physiotherapist in good standing with the College of Physiotherapists of Ontario.
  • In-depth knowledge of physiotherapy services and community resources.
  • Understanding of healthcare professional roles.
  • Relevant community/healthcare experience.
  • Knowledge of physiotherapy tools, equipment, and assistive technology.
  • Understanding of funding agencies related to physiotherapy.
  • Knowledge of relevant legislation (healthcare provision, privacy, safety).
  • Familiarity with practice standards and guidelines set by CPO.
  • Ability to prioritize duties, manage multiple patients, and organize workload efficiently.
  • Strong interpersonal and collaboration skills.
  • Effective listening, observation, and facilitation skills.
  • Excellent communication skills via reports, meetings, and presentations.
  • Proficiency in computer software (email, internet) and Microsoft Office (Teams, Word, Excel).
  • Valid driver’s license and access to a vehicle.
  • Fluent in English, both oral and written.
  • Mandatory COVID-19 vaccination proof prior to start date.

What would give you the edge?

  • Experience with diverse patient groups (multicultural, homeless, palliative, ABI, mental health, geriatrics, pediatrics).
  • Case management or recent community experience.
  • Ability to speak French or another second language.
