Job summary

Scarborough Centre for Healthy Communities (SCHC) is hiring a Physiotherapist for the Community Home Care Team in Ontario. The role focuses on evidence-based assessment, treatment, and education in clients’ homes and community settings, to maximize mobility and independence while supporting aging in place.

The position is 1 FTE (35 hours/week) on a 1-year contract with potential extension, reporting to the Manager, Seniors Support Services. Travel is required throughout the community.

Qualifications

• Degree in Physiotherapy from an accredited university program.
• Current registration in good standing with the College of Physiotherapists of Ontario.
• Current CPR certification.
• Valid “G” License and access to a vehicle required.
• Minimum of three years of physiotherapy experience preferred.
• Experience in community health, home care, geriatrics or primary care is an asset.

• Conduct comprehensive physiotherapy assessments to evaluate mobility, strength, balance, ROM, and function.
• Develop, implement, monitor, and evaluate individualized treatment plans.
• Provide therapeutic exercise programs, manual therapy, mobility training and rehab.

We are currently looking to hire a:

Physiotherapist – Community Home Care Team

1 FTE (35 hours /week) – 1 Year Contract *possible extension*

About the Position

The Physiotherapist is responsible for providing evidence-based assessment, treatment, rehabilitation, and education to individuals living in the community. As a member of the interdisciplinary Community Home Care Team, the Physiotherapist works collaboratively with healthcare professionals to maximize clients' mobility, independence, functional ability, and quality of life while supporting aging in place.

The Physiotherapist develops individualized treatment plans that address mobility limitations, chronic conditions, injury recovery, pain management, balance impairments, and fall prevention. Services are delivered in clients' homes and community settings using a client-centred, holistic approach.

Reporting to: Manager, Seniors Support Services

Key Responsibilities

• Conduct comprehensive physiotherapy assessments to evaluate mobility, strength, balance, range of motion, endurance, posture, gait, and functional independence.
• Develop, implement, monitor, and evaluate individualized treatment plans based on client goals and clinical best practices.
• Provide therapeutic exercise programs, manual therapy, mobility training, and functional rehabilitation.
• Assess and manage clients recovering from illness, injury, surgery, or hospitalization.
• Provide treatment for clients with neurological, musculoskeletal, cardiovascular, and age-related conditions.
• Promote pain management strategies through therapeutic interventions and client education.
• Reassess client progress and modify treatment plans as required.

Falls Prevention and Mobility

• Conduct falls risk assessments and implement individualized fall prevention strategies.
• Recommend exercises to improve balance, coordination, flexibility, and strength.
• Assess gait and mobility and provide recommendations for safe ambulation.
• Recommend and educate clients on the safe use of mobility aids and adaptive equipment.
• Collaborate with Occupational Therapists regarding home safety modifications and equipment needs.

Client and Caregiver Education

• Educate clients and caregivers on exercise programs, injury prevention, body mechanics, and mobility strategies.
• Encourage self-management techniques that support long-term independence.
• Provide education on maintaining physical activity and healthy aging.

Care Coordination

• Collaborate with Registered Nurses, Personal Support Workers, and other healthcare providers to develop integrated care plans.
• Participate in interdisciplinary case conferences and team meetings.
• Coordinate referrals to community rehabilitation and specialized therapy services when appropriate.
• Communicate client progress and recommendations to the interdisciplinary team.

Documentation and Quality Improvement

• Maintain accurate, timely, and confidential clinical documentation in the electronic medical record.
• Complete assessments, progress notes, treatment plans, and discharge summaries in accordance with organizational and professional standards.
• Participate in quality improvement initiatives and program evaluation activities.
• Monitor client outcomes and recommend service enhancements.

Qualifications

Experience

• Experience working with older adults, individuals with chronic disease, and clients with complex medical needs.
• Experience working within an interdisciplinary healthcare team.

Knowledge and Skills

• Strong clinical assessment and rehabilitation skills.
• Knowledge of evidence‑based physiotherapy practices and rehabilitation techniques.
• Experience with mobility assessments, balance training, and fall prevention programs.
• Excellent communication, coaching, and motivational skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Strong organizational, time management, and problem‑solving skills.
• Proficiency with electronic medical records and Microsoft Office applications.

Working Conditions

• Valid Ontario driver's licence and access to a reliable vehicle is required.
• Travel throughout the community to clients' homes and community locations required.
• Flexible work schedule- due to the nature of community-based healthcare services, the successful candidate will be required to work rotational weekends as scheduled to ensure continuity of care and service delivery.
