Adelanto Community Health Center - Adelanto, California

Ontario Health Center - Ontario, California

• A Federally Qualified Health Center, this site is eligible for Public Student Loan Forgiveness (PSLF).
• Referral resources include WIC, Dental, Psych, Nutritionist, and a MediCal case worker.
• Facility hours for this location are Mon-Fri 8am-5pm.
• Good mix of patients including peds.
• 15-18 patients per day per provider.
• Point of Care testing available at site; minimal labs.
