Job description

Lifemark Health provides comprehensive rehabilitation-focused assessment and intervention for patients experiencing barriers to transitioning to life after service due to service-related injuries. Our interdisciplinary team includes clinicians with expertise in managing complex conditions such as chronic pain, head injury symptoms, MSK injuries, often with comorbid mental health issues and psychosocial barriers to recovery.

Our clients come from sources including Worksafe BC, Partners in Canadian Veterans Rehabilitation Services (PCVRS), and others.

As part of the interdisciplinary team, the Physician shall use evidence-based and measurement-based care to perform the following:

  1. Complete thorough assessments of eligible health problems;
  2. Perform medication reviews and identify drug therapy problems;
  3. Clarify relevant medical issues, including comorbid conditions;
  4. Determine diagnosis and prognosis related to health problems;
  5. Identify unrelated conditions that may hinder rehabilitation progress if unaddressed;
  6. Identify relevant accommodations or medical restrictions;
  7. Recommend additional specialized assessments when needed to achieve rehabilitation goals;
  8. Collaborate on treatment plans emphasizing education, coping, and self-management to improve function and recovery;
  9. Engage with patients, support persons, and family members to promote understanding of medical conditions and recommendations;
  10. Communicate with the patient’s primary healthcare provider to coordinate care;
  11. Provide care in-person or virtually;
  12. Participate in debriefs with the interdisciplinary team as needed;
  13. Conduct targeted follow-up care within PCVRS;
  14. Collaborate with PCVRS Medical Directors and engage in quality improvement, learning, and skills development.

Requirements:

  • Doctor of Medicine with a valid license and good standing with the provincial regulatory body;
  • Experience managing complex medical conditions;
  • Excellent verbal and written communication skills;
  • Experience with military culture and Canadian patients is an asset;
  • Bilingual (French/English) is an asset;
  • Experience working with multidisciplinary teams is an asset.
