This role is eligible for a sign-on bonus of up to $20,000.00

This is a full-time permanent role located at 4300 Wellington Road, Suite 101, Nanaimo, British Columbia.

This role is eligible for up to a $10,000 signing bonus

• You will work with clients to develop and implement individualized treatment plans, aimed at maintaining and enhancing their physical abilities, lifestyle, and well-being.
• You will assess and deliver direct client care, handle documentation, and communicate effectively to support case management.
• You will participate in a collaborative care model providing holistic, comprehensive care to meet client needs.
• You will promote positive relationships between clients, team members, funders, and the community.

Salary Range: $50.00-$150.00 per hour (commission pay model)
