Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing patient care needs related to mobility, endurance, pain, and other medical conditions. Based on assessments, the therapist develops treatment plans, performs interventions to improve well-being, and evaluates patient progress.

  1. Evaluate the patient's functional status and needs, assisting in developing the therapy plan of care.
  2. Monitor and report patient responses and condition changes to the healthcare team.
  3. Treat patients to relieve pain, restore function, and maintain maximum functioning according to standards of practice.
  4. Educate patients, families, and healthcare team members about the care plan, limitations, and progress.
Education and Experience

Licensure Requirements:

  • Current Physical Therapy license in Florida
  • Current CPR certification
  • Valid driver’s license, vehicle insurance, and access to a dependable vehicle
