Home health visits without the travel! This position covers Senior Living Communities (ALF/ILF) only, providing home health services.

$2,500 SIGN ON BONUS FOR FULL-TIME

Prior managerial experience is a plus.

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a per diem, part-time, or full-time home health visit role in the Harrisonburg, VA area, serving adult and geriatric clients.

As a home care Physical Therapist, you will be part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities include:

1. Make home visits in designated territories.
2. Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and revise individualized care plans, including manual exercises, gait training, and balance interventions.
5. Assess and update care plans, collaborating with nursing staff as needed.
6. Educate clients and families on rehabilitative activities to promote health and independence.
7. Document observations and interventions accurately using electronic tablets.

Qualifications:

• Current VA Physical Therapist license.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Computer proficiency; EMR experience preferred.
