Overview

PeaceHealth Home Health in Bellingham, WA is seeking a Physical Therapist for a Per Diem/Relief, 0.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $45.14 – $67.70. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

Responsibilities
  • Evaluates assigned patients and establishes a treatment plan and goals, using data from a variety of sources, such as patient records, interviews, observation, and team members.

  • Provides patient treatment according to plan of care and modifies plan to meet patient needs, and updates plan as appropriate.

  • Completes documentation accurately, timely, according to regulatory and state licensure requirements, and within professional standards.

  • Collaborates with other healthcare professionals as necessary to ensure smooth continuous care of the patients. May act as a liaison with nursing, medical and other therapy staffs to facilitate problem solving and coordination of other services and act as an educational resource.

  • May mentor and provide oversight to new caregivers, physical therapist assistants, and/or therapy aides. Provides clinical instruction and training to students.

  • Recommends and participates in development of new services and programs, quality improvement and monitoring activities.

  • Makes recommendations for relative content and scope of therapy services and implements policies and procedures that guide and support the provision of services.

  • Recommends and develops new therapy services and programs in collaboration with staff.

  • May spend minimal time to assist with evaluating and orienting new caregivers and making recommendations to management regarding equipment and space needs related to physical therapy functions.

Qualifications
  • Bachelor's, Master's or Doctorate Degree Required: Physical Therapy accredited by the Commission on Accreditation in Physical Therapy Education or closely related field (foreign equivalent accepted)

  • Required: Physical Therapist License State of Washington

  • Required Upon Hire: Basic Life Support

  • Valid driver’s license and proof of auto insurance.
