Job summary

Premier Home Health Providers Inc. is seeking a Physical Therapist to deliver skilled home health care on a per diem basis. You will assess patients, create individualized plans, and provide therapy in a home setting, following physician orders and CA state practice acts.

Colleagues will collaborate with physicians, care teams and families, ensuring smooth transitions from facility to home. A valid CA PT license, CPR certification, and vehicle are required; 2+ years of PT experience is preferred.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
