Job summary

Premier Home Health Providers Inc. is seeking a licensed Occupational Therapist to deliver home-based OT services on a per diem basis. You will assess patients, develop individualized plans, educate families, and coordinate with the care team in patients' homes.

This role requires active California OT license, 2+ years of clinical OT experience, CPR certification, and reliable transportation. Flexible scheduling, mileage reimbursement, and potential pathway to full-time employment are offered.

Qualifications

  • OT degree accredited by the appropriate bodies.
  • Registered to take the AOTA National Registration Examination.
  • Active California Occupational Therapy license.
  • Minimum 2 years of clinical OT experience.
  • CPR certification (AHA).
  • Valid driver’s license and ability to travel to patient homes.
  • Strong communication and organizational skills.

Responsibilities

  • Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
  • Develop personalized treatment plans and communicate them to physicians and the care team.
  • Teach and retrain patients in activities of daily living affected by illness or injury.
  • Recommend adaptive equipment, splints, and prosthetic/orthotic devices.
  • Maintain accurate clinical records and promptly report changes in patient condition.
  • Coordinate with the broader care team to ensure seamless, patient-centered care.
  • Educate family members and caregivers on the patient’s treatment approach.
  • Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service.
  • Support discharge planning as patients transition out of home health care.
  • Perform other duties as required for patient services.

  • Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
  • Registered with the National Registration Examination of the American Occupational Therapy Association
  • Active California Occupational Therapy license
  • Minimum 2 years of clinical OT experience
  • Current CPR certification with AHA
  • Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
