Job summary

Humber River Health Pharmacy department is seeking a Pharmacy Intern for Fall 2023, with preceptors to enable completing internship requirements. The intern will work in a hospital setting, averaging 5 days per week, on a temporary full-time basis, with shifts including weekends and holidays.

The role involves provision of drug therapy under supervision, pharmacotherapy monitoring, medication history documentation, and various drug-use control tasks, in MEDITECH.

Qualifications

• Completion of University of Toronto or University of Waterloo Undergraduate Pharmacy Program by June 2023.
• Hospital pharmacy experience an asset.
• Demonstrated ability to work independently and as a member of a team.
• Proven ability to prioritize a consistent and varied workload.
• Good written and verbal communication skills required, in order to interact effectively with patients, peers, and other health care team members.
• Demonstrated ability to identify, prevent and resolve actual and/or potential drug related problems.
• Effective interpersonal skills.
• Excellent attendance and discipline free record required.
• An understanding of equity, diversity, and inclusion principles as they relate to health practices is an asset.

Responsibilities

• Provision of drug therapy, under supervision of a Pharmacist, for the purpose of achieving definite outcomes to improve patient’s quality of life.
• Clinical pharmacotherapy monitoring, drug information and adverse reaction reporting.
• Review of physician’s orders, computer entry of prescriptions in the MEDITECH system, creation and maintenance of patient medication profiles.
• Other “drug use control” functions which include checking of chemotherapy
• Quality assurance projects
• Best Possible Medication Histories and Medication Reconciliation
