Detailed Overview

The Pharmacist provides distribution pharmacy services by interpreting medication orders/prescriptions, assessing prescriptions for therapeutic suitability, and ensuring medications are efficiently and accurately dispensed, compounded, labeled, and records are complete. Communicates with physicians, patients, and other healthcare team members for prescription clarification and problem resolution. Provides instructions on proper use/administration, storage, disposal, and possible side effects of medications. Participates in medication use evaluation projects, medication safety initiatives, quality control activities, contributes to publications, and participates in clinical drug trials in accordance with established standards and procedures.

Additionally, the Pharmacist functions as a Clinical Pharmacist responsible for resolving select patient drug-related problems, participating in ward rounds, and documenting therapeutic plans.

Responsibilities

  • Interprets medication orders and dispenses medications using a computerized system in accordance with policies and standards.
  • Assesses prescriptions for therapeutic suitability, referring to references, laboratory data, and patient demographics.
  • Reviews patient profiles for allergies, drug interactions, and contraindications, withholding medications if safety concerns arise.
  • Communicates with healthcare professionals for prescription clarification and documents interactions.
  • Provides instructions on medication use, storage, disposal, and side effects, communicating effectively with patients and healthcare providers.
  • Participates in medication history collection, allergy verification, and medication reconciliation.
  • Ensures medications are dispensed, compounded, and labeled accurately, with complete records.
  • Provides medication counseling and discharge instructions, documenting interactions appropriately.
  • Reviews reports for drug-related problems and provides pharmacokinetic interpretations for targeted drugs.
  • Participates in medication use evaluations, inventory management, and quality improvement activities.
  • Maintains records for controlled substances and participates in safety initiatives and staff education.
  • Contributes to publications, clinical trials, and reports adverse reactions.
  • Attends meetings and committees, seeking process improvements.
  • Functions as a clinical pharmacist in ward rounds and therapeutic planning.
  • Performs other duties as assigned.

Qualifications

Education and Experience
Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
