Reports to : Clinical Site Leader

Hours : 37.5 Hours per Week

Shifts : Mon-Fri Days / Evenings / Weekends / Statutory holidays

Status : Permanent Full-Time

Position Summary

In this position the Pharmacist provides comprehensive pharmacy services to ambulatory chemotherapy / systemic therapy patients and provides centralized pharmacy services to inpatients within the PM Cancer Centre Program. You will be expected to facilitate and support clinical teaching and research activities within the department while exemplifying UHNs values of caring excellence teamwork innovation integrity leadership and respect with demonstrated teamwork in a patient care setting.

Duties

  • Review verify and dispense all ambulatory standard-of-care and clinicaltrials oncology medication orders for appropriateness
  • Identify resolve and prevent real or potential drug related problems
  • Monitor drug therapy for effectiveness toxicity and adherence to evidence based medicine / guidelines
  • Conduct medication reconciliation or patient counselling on a consult / referral basis
  • Follow up on medication incidents and promote safe medication practices on the unit
  • Respond to drug information questions
  • Be active in the pharmacy student preceptorship in the undergraduate pharmacy programs as well as providing mentorship and preceptorship for the pharmacy residency program at UHN
  • Providing direct and / or functional supervision of technician staff
  • Performing other duties consistent with job classification as required

Qualifications :

  • Completion of university degree (Bachelors of Science Degree in Pharmacy) or PharmD or equivalent required
  • Certificate of competency from the Pharmacy Examination Board of Canada required
  • Active registration with the Ontario College of Pharmacists (Part A) required
  • Hospital pharmacy practice experience in oncology preferred
  • Hospital pharmacy residency an asset
  • Clinical experience with patient-centered care in oncology would be an asset
  • Membership in the Canadian Society of Hospital Pharmacists an asset
  • Excellent verbal and written communication skills
  • Excellent interpersonal skills
