Qualifications

Position Summary

Pharmacist
Your goal in this position is to provide direct patient care to identify, solve, and prevent drug therapy problems based on an evidence-informed practice. You will also participate in patient education, support the teaching of students, provide drug information and/or education to nursing and medical staff, and ensure patient medication safety by supporting drug distribution services. You are a great relationship builder, communicate effectively with peers across the entire organization, and effortlessly connect with physicians, staff, patients, and families in our diverse community. You are highly organized, have great interpersonal skills, demonstrate critical thinking and reasoning skills, and adapt well in a dynamic environment. You will take on the shared responsibility for ensuring that health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers, and families.

On a practical level, you will
  1. Provide patient-centred clinical services – medication history, medication therapy management, pharmacotherapy monitoring, drug information, drug use evaluation, adverse drug reaction reporting, medication counseling, medication incident reporting and follow-up.
  2. Work collaboratively within an interprofessional team.
  3. Participate in educational and teaching activities within the department, on the nursing units, and throughout the Hospital.
  4. Maintain activities in all drug use control functions - drug distribution systems, computerized medication order processing system, formulary system, inventory control system, and drug procurement.
  5. Participate in quality of care, research, and patient safety initiatives.
  6. Supervise and provide direction for the activities of pharmacy technicians.
  7. Follow all Hospital and governmental (Federal and Provincial) rules and regulations set to control the dispensing to the general public and the distribution of medications, and to provide high-quality services within the Hospital.
  8. Be responsible for working safely in accordance with the Occupational Health and Safety Act.
Qualifications
  1. Graduate from an accredited university Pharmacy Program.
  2. Licensed under Part A of the Ontario College of Pharmacists.
  3. Hospital residency or hospital direct patient care experience an asset.
  4. Knowledge of pharmaceutical care process, disease management, and clinical pharmacology.
  5. Familiarity with evidence-informed practice.
  6. Demonstrated skills in computerized pharmacy systems.

This is a Part Time Casual position, with 8 hour shifts, day & evening shifts including weekends in Pharmacy Services. This position also includes shifts at the RCC Site at 555 Finch Avenue West.
