The Opportunity

The Pharmacist monitors medication use, provides clinical support, and collaborates with healthcare professionals to ensure safe, effective patient care.

Duties and Responsibilities
  • Complete patient medication assessments
  • Ensure optimal medication therapy and safety
  • Review lab info and oversee drug monitoring
  • Participate in clinical rounds
  • Verify physician orders for drug distribution
  • Assess non-formulary medication requests
  • Provide drug information to healthcare providers
  • Review and recommend on clinical documents
  • Collaborate with interdisciplinary teams
  • Conduct medication safety audits
  • Participate in antimicrobial stewardship and staff education
  • Contribute to quality improvement
  • Other duties as assigned
Education and Experience
  • Registered Pharmacist with Ontario College of Pharmacists
  • BSc in Pharmacy or PharmD
  • Post-grad Residency (asset)
  • Hospital experience (preferred)
  • Experience with MEDITECH Expanse and BD Pyxis (asset)
Competencies
  • Knowledge of pharmacotherapeutics and pharmacokinetics
  • CPD as per Regulated Health Professions Act
  • Understanding of medication dosages and administration routes
  • Ability to interpret lab and clinical data
  • Proficiency with electronic resources and MS Office
  • Research skills and critical appraisal
  • Time management and organization
  • Independent practice capability
  • Problem-solving, communication, and conflict resolution skills
  • Additional language skills are an asset
  • Team collaboration skills

More Information

Location: Pharmacy

Employment Type: Permanent Part-Time

Start Date: In line with next hospital orientation

Hours: Days, evenings, weekends, including all shifts

Wage: $56.41 - $66.99/hr + shift premiums

  • Evening: +$2.26/hr
  • Night: +$2.98/hr
  • Weekend: +$3.14/hr
