Position Summary

Pharmacist, Medication Safety Your goal in this position is to facilitate medication safety initiatives as directed by national and provincial accreditation standards, evaluate recommendations from the Institute for Safe Medication Practices (ISMP) and Health Canada, and assist the pharmacy manager(s) to ensure compliance with safe medication practices at NYGH. Reporting to the Pharmacy Operations Manager, the Medication Safety Pharmacist works in collaboration with Clinical Team Managers and Patient Safety and Risk Specialist(s) to investigate patient safety events (SLIPs) and participates in the development of recommendations and action plans in improving the medication use system in the organization, with the goal to minimize risk of medication incidents. Additionally, the Medication Safety Pharmacist provides case(s) debriefing and compiles trending reports of medication-related incident data to the Pharmacy & Therapeutics Committee (and to the Quality and Patient Safety Committee and Medical Advisory Committee (MAC) as requested), reviews adverse drug reaction reports, and supports the review of policies and procedures related to medication safety. The Medication Safety Pharmacist also provides direct patient care to identify, solve, and prevent drug therapy problems based on evidence-informed practice. You will also participate in patient education, support the teaching of students, provide drug information and/or education to nursing and medical staff, and ensure patient medication safety by supporting drug distribution services. You are a great relationship builder and can communicate effectively with peers across the entire organization and effortlessly connect with physicians, staff, patients, and families. You are highly organized, have great interpersonal skills, demonstrate critical thinking and reasoning skills, and adapt well in a dynamic environment. This position supports the hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, learners, volunteers, and families.

On a practical level, you will

Perform Medication Safety Related Duties

  • Review, analyze and trend medication-related reports (SLIP). Categorizes incidents according to severity and type of incident, performs root cause analyses in collaboration with Patient Safety and Risk Specialist(s), and assists the Pharmacy Operation Manager or Clinical Team Managers in developing corrective action plan(s) to prevent future recurrence of similar incidents as applicable.
  • In collaboration with the pharmacy manager, develop recommendations for operational and system improvements in medication use to reduce risk of future errors based upon findings in accordance with principles of quality improvement as a member of relevant incident review working group(s) and coordinates medication safety projects within the department and across NYGH as required.
  • Support hospital-wide patient safety initiatives as an active member on the Medication Use and Safety Committee, Drug Diversion Committee, Pharmacy and Therapeutics Committee, and other working groups or committees as assigned.
  • Support the development and/or revision of medication safety-related policies, medical directives, procedures, and protocols at NYGH.
  • Collaborate with the Patient Safety and Risk Specialist(s) in promoting the importance of safety event reporting throughout the organization via a non-punitive incident reporting system (SLIP).
  • Report eligible medication incidents and near misses to the Institute for Safe Medication Practices (ISMP Canada) and National System for Incident Reporting (NSIR) Centre as required.
  • Present information of selected high-risk cases of medication-related incident reported in SLIP and provide a detailed quarterly analysis of submitted SLIP reports at the Medication Use and Safety Committee, Pharmacy and Therapeutics Committee, Quality and Patient Safety Committee (and any other committee as requested) with written briefing summary.
  • Support Patient Safety and Risk Specialist(s) in collecting information on adverse drug reactions (ADR) identified by staff and in preparing the ADR report to be submitted to Health Canada in accordance with NYGH policy and Vanessa’s Law (Protecting Canadians from Unsafe Drugs Act).
  • Review the latest publications from ISMP and other medication safety bodies, communicate any relevant safety alerts or concerns to pharmacy team members, Patient Safety and Risk Specialist(s) and other stakeholders, and evaluates if any of these published documents identify potential medication safety risks in NYGH.
  • Serve as a key resource related to medication safety-related issues for all clinical areas, and monitor the compliance with medication management standards set by Accreditation Canada or Ontario College of Pharmacists.
  • Facilitates periodic self-assessments of medication safety using ISMP medication safety self-assessment tool (MSSAs) and conducts root cause analysis (RCA) studies of incidents and failure mode and effects analysis (FMEA) of high-risk processes.
  • Collaborate in the development of the Medication Safety dashboard, monitor and validate the safety metrics on the dashboard as needed.
  • Participate in any relevant Quality Improvement Plan (QIP) initiatives related to medication safety, such as evaluating infusion pump dose error reduction library use compliance and override rate. Perform Direct Patient Care
  • Provide medication management consultation services and other non-medication distribution activities for the Maternal Newborn Program. The clinical practice should align with the Clinical Pharmacist (Inpatient) job description. The clinical portfolio assignment can be changed to accommodate pharmacy operational requirements as needed.
  • Provide patient-centered clinical services – medication history service, medication order review and assessment, pharmacotherapy monitoring, inter-professional participation, drug use evaluation, adverse drug reaction reporting, medication counseling, medication incident reporting and follow-up.
  • Maintain activities in all drug use control functions-drug distribution systems, computerized medication order processing system, formulary system, inventory control system and drug procurement. Provide Teaching and Educational Activities
  • Participate in drug information and other educational and teaching activities within the department, on the nursing units and throughout the Hospital through written and oral communication.
  • Participate in quality of care, research and patient safety initiatives.
  • Support the training of pharmacy students or pharmacy residents by precepting rotations in medication safety
  • Design posters and/or implement educational presentations that facilitate the understanding and implementation of medication safety standards at NYGH.
  • Responsible for the activities of the pharmacy technicians assigned on a day to day basis.
  • Follow all Hospital and governmental (Federal and Provincial) rules and regulations set to control the dispensing to the general public and the distribution of medications, and to provide high quality services within the Hospital.
  • Responsible for working safely in accordance with the Occupational Health and Safety Act.

Qualifications

  • Graduate from an accredited university Pharmacy Program
  • Licensed under Part A of the Ontario College of Pharmacists
  • Previous clinical hospital experience or hospital residency is an asset
  • Three to five years of recent inpatient hospital pharmacy experience
  • Knowledge of pharmaceutical care process, disease management and clinical pharmacology
  • Familiarity with evidence-informed practice
  • Demonstrated skills in computerized pharmacy systems (Oracle Cerner)
  • Bilingualism in French/English an asset Skills Required
  • Well-developed critical appraisal and critical thinking skills, and a detail-oriented and strong analytical skill to evaluate medication safety data and to identify patterns, risks, root causes, and potential improvement opportunities.
  • Advanced computer skills including database management; experience with Oracle Cerner is an asset.
  • Good interpersonal and conflict management skills to interact as an effective team member to foster a collaborative working environment.
  • Ability to work independently, organize work and establish priorities.
  • Demonstrate ability to proactively resolve problems and conflicts.
  • Excellent presentation and communication skills, both written and verbal including patient counseling skills.
  • Demonstrated expertise in the provision of pharmaceutical care.
  • Demonstrated commitment to ongoing learning and professional development.
  • Previous experience with medication safety initiatives preferred.
  • Experience with data analysis and effective monitoring and evaluation of implemented programs.
