This is role is a 12-18 month mat leave cover. The successful candidate will be working within our Pain and Medication Management Program located at either our Victoria Discovery or Surrey Fleetwood clinic and supporting the other clinic virtually.

Your Role:

As a Pharmacist, you will be responsible for:

  • Medication management: Conduct assessments with respect to clients’ presenting condition, current and past medication use (both prescription and non-prescription), benefits and side effects of medications with the optimization of the PhamaNet record.
  • Medication education for both clients and clinical team as needed.
  • Support team physician in prescribing and monitoring of any necessary medications relevant to the patient’s rehabilitation needs.
  • Evaluate new medication information and its relevance and appropriateness to the practice of chronic complex pain and rehabilitation with a focus on the biopsychosocial model of treatment.
  • Problem solves with team members regarding needs of the client including specific needs resulting from medication tapers or maintenance.
  • Ensure all documentation in the clinical record meets CBI Health and regulatory body standards.
  • Ensure accurate, concise, and relevant reporting to stakeholders occurs as per expectations.

What you will need to be successful:

  • Be able to work both autonomously and as part of an interdisciplinary healthcare team.
  • Be efficient and responsive in the provision of required clinical documentation.
  • Demonstrate a work ethic, reliability, maturity, and flexibility.
  • Hold a pharmacy degree from an accredited university pharmacy program recognized by the Canadian Pharmacists Association of Canada (CPhA).
  • Hold a pharmacy license with the College of Pharmacists of British Columbia.
