Duties

  1. Coordinate with pharmacy staff to ensure the proper flow of medication from procurement to dispensing and administration.
  2. Lead and mentor pharmacy team members.
  3. Participate in quality improvement initiatives and ensure compliance with pharmacy standards and regulations.
  4. Support medication management activities including reconciliation, safety, and interdisciplinary collaboration.
  5. Teach and mentor students and learners.
  6. Collaborate with healthcare professionals to optimize patient care.
  7. Disseminate drug information and participate in formulary reviews.
  8. Provide flexible pharmaceutical care across clinical areas, including day shifts and on-call duties.
  9. Adhere to safety practices and promote an anti-racism, discrimination, and harassment-free workplace.
  10. Perform other duties as assigned by the Director of Pharmacy.

Qualifications

  1. Bachelor of Science in Pharmacy or equivalent, with registration in good standing with the Ontario College of Pharmacists.
  2. Hospital residency or PharmD postgraduate training is an asset.
  3. Experience with CPOE, BMV, eMedRec, eMAR, EMR, and Meditech software is preferred.
  4. Knowledge of hospital care and drug therapy, with experience in medication reconciliation.
  5. Good interpersonal, communication, and organizational skills.
  6. English proficiency is mandatory; bilingual in French/English is an asset.
