Job summary

Sunnybrook Health Sciences Centre is seeking a Pharmacist to support acute care pharmacy, including direct patient care and liaison work with the CrCU. The role involves physician order review, dispensing supervision, medication reconciliation, and participation in multidisciplinary rounds.

Ideal candidates will hold current licensure with the Ontario College of Pharmacists and have prior acute care hospital experience, with residency or critical care exposure preferred.

Qualifications

Responsibilities

• Oversee automated dispensing unit cabinets; participate in safety reporting.
• Engage in education of students and staff; participate in hospital and departmental committees.

Summary of Duties

We have avacancy for a Pharmacistin the Acute Care Pharmacy.This Pharmacist 1 role will have liaison responsibilities to the CrCU.

The pharmacy duties can be broadly divided into three areas:

1. Drug distribution
2. Direct patient care.
3. Educational responsibilities

Responsibilities include physician order review; clarification and entry; drug dispensing and or supervision of such tasks by pharmacy technicians; answering of drug-related questions; provision of pharmaceutical care; medication reconciliation; participation in multidisciplinary rounds; patient counselling; participation in the education of students and multidisciplinary staff; oversight of automated dispensing unit cabinet; safety report initiation and follow-up; other pharmacy duties including participation in on-call activities and hospital and departmental committees.

Qualifications/Skills

• Current licensure by the Ontario College of Pharmacists
• Recent previous Acute Care Pharmacy experience and hospital pharmacy residency preferred
• Experience working in critical care preferred
• Able to work well in a fast-paced, sometimes stressful work environment
• Self-motivated and able to take initiative
• Must work effectively with minimum of direction and within a team setting
• Demonstrated ability to work well in a multidisciplinary team environment
