Job Summary

Reporting to the Manager, Clinical Pharmacy Services, the Clinical Pharmacist is an integral member of the Pharmacy Team. The Clinical Pharmacist is accountable for completing patient medication assessments, ensuring optimal medication therapy, and minimizing potential for adverse events. They provide accurate medication information to the interdisciplinary team and promote safe and appropriate use of medications in a multidisciplinary setting.

Responsibilities:
  • Complete patient medication assessments.
  • Ensure optimal medication therapy and minimization of potential adverse events.
  • Review lab information to ensure appropriate drug therapy.
  • Participate in patient rounds in clinical areas.
  • Participate in drug distribution, including accurate order entry of physician orders.
  • Provide relevant drug information to pharmacists and other healthcare professionals.
  • Review and provide recommendations on various document submissions.
  • Work collaboratively with the interdisciplinary team on patient care units.
  • Complete appropriate medication safety audits.
  • Participate in the education and mentoring of new staff and precepting students.
  • Participate in the ongoing education of nursing staff.
  • Other duties as assigned.

Education and Licensing:
  • Registered Pharmacist in good standing with the Ontario College of Pharmacists (Part A).
  • Bachelor of Science or PharmD degree in Pharmacy.
Experience:
  • Post-graduate Pharmacy Residency Program (ACPR), an asset.
  • Minimum two (2) years of experience in a hospital pharmacy setting, preferred.
  • Knowledge of current evidence-based pharmacotherapeutics & pharmacokinetics.
  • General knowledge of disease states, such as diabetes, COPD, and cardiovascular disease.
  • General knowledge of appropriate dosages, delivery techniques, and formulations.
  • Knowledge of lab and clinical parameters for patient assessment.
  • Knowledge of credible medical information resources.
  • Knowledge of third-party reimbursement strategies.
  • Knowledge of health promotion and disease prevention.
  • Knowledge of complementary medicines and alternative therapies.
  • General knowledge of aseptic technique and preparation of pharmaceuticals.
  • Knowledge of multiculturalism and diversity.
  • Knowledge of Microsoft applications.
  • Understanding of research methodology and critical appraisal of study design.
  • Excellent time management and organizational skills.
  • Ability to function as an independent practitioner.
  • Sound problem-solving, communication, conflict resolution, and decision-making skills.
  • Highly motivated team player with collaboration skills.
  • Ability to perform multiple tasks and work under pressure.
  • Excellent attendance, punctuality, and employment record.
