Job summary

CBI Home Health in St. Catharines, ON is seeking a Personal Support Worker on a 6-month guaranteed hours contract. The role is part-time, with a rotating schedule across North St. Catharine's and Thorold/South St. Catharine's, delivering compassionate in-home care.

You will provide personal care, assist with mobility, and document changes in client condition. You must have a PSW certificate, First Aid/CPR Level C, and be able to travel between assigned regions.

Qualifications

Responsibilities

• Support clients to be as safe and independent in their homes as possible.

• Observe, document, and report changes in client condition to the Manager/Supervisor.
• May perform light housekeeping tasks as indicated in care plans.

Travel region : North St. Catharine's

Travel region : Thorold/South St. Catharine's

Salary Range: The salary range for this position is $22.89 – $24.63 per hour.

Vacancy Disclosure: This posting is for an existing vacancy with guaranteed hours for a period of 6 months contract. Incumbent should commit to full 6 month assignment.

Morning Rotations #1 - North St. Catharine's (Bi-weekly hours 35 hrs)

Week 1: Monday, Tuesday, Saturday and Sunday (7am to 12pm) with Wednesday, Thursday and Friday Off

Week 2: Wednesday, Thursday, Friday (7am to 12pm) with Monday, Tuesday, Saturday and Sunday Off

Morning Rotations #2 - North St. Catharine's (Bi-weekly hours 35 hrs)

What you will do

• You will support clients to be as safe and independent in their homes as possible
• You will provide personal care to clients in their home, and may assist them to mobilize and perform light exercises.
• You will act at all times with the client and their family at the heart of all you do, observing, documenting, and reporting any changes in their condition to your Manager/Supervisor.
• You may be required to perform light housekeeping tasks as indicated in client care plans.

What you will need to be successful

• Your Personal Support Worker Certificate or equivalent
  • Equivalencies
    • Home Support Worker Level I, II or III, Health Care Aid, Personal Attendant (PA+2), Developmental Support Worker, OTA/PTA, Paramedic
    • Internationally Trained Registered Nurse/ Registered Practical Nurse (RN/ RPN), Midwife or Physicians
    • Students currently enrolled post 1st year in Nursing, Rehab, Medicine or Paramedicine programs
  • Valid First Aid and CPR certification, Level C
  • A clear Criminal Record Check with Vulnerable Sector Screening within the last 12 months
  • Proof of a negative TB test within the last 12 months and an up-to-date immunization record
  • Ability to travel from home to home within the community
  • Ability to assist with two-person client transfers involving individuals with varying levels of mobility and complexity.
  • Availability to work all scheduled days and times withing the assignment. The individual who holds this rotation has the option to provide additional availability outside of this assignment to complete additional client visits if they wish, provided such work is available.
  • Commitment to the full 6-month assignment
  • Strong verbal and written English communication skills required; the ability to communicate in French or other languages is considered an asset
  • Reliability and strong organization skills
  • Ability to safely lift up to 35 lbs. (16 kg) on a consistent basis throughout the workday.
  • Flexibility to complete to same-day, last minute book-offs within the designated geographic areas and hours of work of this assignment.
  • Access to a reliable personal vehicle is required.
  • Willingness to complete required training, including (but may not be limited to) Safe Patient Handling.
