Job Summary

Our Personal Support Workers (PSWs) report to the Client Services Coordinator and are expected to deliver a high standard of care that allows our clients and patients to remain safe and comfortable in their own home environment. The services provided will be detailed in the personalized care plan. We are currently recruiting PSWs for York Region, particularly in Stouffville and Markham.

Responsibilities

• Assist clients with personal care, activities of daily living, meal preparation, medication reminders, light housekeeping, and light laundry.
• Accompany clients on outings, appointments, shopping, and errands where applicable.
• Provide companionship and respite for families.
• Maintain daily documentation and charting in electronic systems on changes in clients’ health or environment.
• Foster a safe, respectful, and inclusive environment for all clients.
• Communicate effectively with clients, families, and healthcare teams.

Qualifications

• Proficient communication in English (verbal and written); additional languages are assets.
• Ability to travel to clients via transit or car.
• Personal Support Worker Certificate from a recognized Ontario program or equivalent (or related healthcare certification).
• Previous PSW experience is an asset.
• Access to internet and mobile phone.
• Negative TB skin test or chest x-ray within the last 6 months.
• Current CPR and First Aid certification.
• Vulnerable Sector Screen completed within 12 months.
• Vaccinations: Covid-19, MMR, Tdap, Polio, Varicella, Hepatitis B.
