Job Description

Partners Community Health (PCH) is a new not-for-profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH currently operates two new state-of the-art LTC homes and community hub in West Mississauga. The new homes have a combined 632-beds and be part of PCH’s larger strategy that introduce innovative and inclusive programs and services and new models of care delivery. PCH is committed to building partnerships and connections that put people first, as well as a learning and leading healthcare community that provides best in class service across the aging continuum.

Key Responsibilities:

  • Assume responsibility for provision of Quality Resident Care on the resident home area (RHA) as per PCH’s policies and accountability framework
  • Support residents/families by communicating with them and involving them in the provision of their own care when possible
  • Participate in all aspects of personal resident care to ensure the physical, psychological, social, and spiritual needs of each resident/family are met under the direction of the Registered Staff and/or Director of Resident Care
  • Utilize therapeutic communication by establishing a working relationship with resident/families and co-workers for the purpose of meeting the resident’s needs, ensuring respect, dignity, individuality of each resident and staff member
  • Deliver effective and efficient resident-focused care that promotes resident/family choices and acknowledges the resident’s strength, limitations as well as their need for safety
  • Participate with other team members in formulating, delivering and reporting the daily resident care needs and preferences as per the individual’s plan of care
  • Participate in continuous quality improvement of the RHA and associated care services
  • Participate in mandatory education/training in-services for skill development and updates on new and/or relevant information on best practices

Qualifications:

  • Current Personal Support Worker Certificate from a recognized institute
  • Completion of six-hundred and forty (640) hours in PSW curriculum
  • Minimum 1 year experience in a Long-Term Care setting an asset
  • Knowledge of Ministry of Health and Long-Term Care (MOHLTC) standards
  • Ability to work well with others in a team approach and adapt to changing situations
  • Ability to work effectively in a multidisciplinary stressful environment
  • Excellent communication skills (written and oral), interpersonal skills and computer literacy
  • Ability to demonstrate an anti-oppression approach to practice

Job Requirements:

  • PSW Diploma/Certificate
  • Valid First Aid & CPR Certification
  • Must present a clear Vulnerable Sector Check (completed in the last 6 months)
  • Completed 2-Step TB Test
  • Must be available to work weekends and statutory holidays
