Job Summary:
The Personal Support Worker provides personal support and homemaking services to clients and families within the framework of VON policies, procedures, and standards. The PSW collaborates with members of the health care team to meet client needs and attain an optimal level of independence and well-being.
This position is eligible for the temporary $3.18 per hour wage enhancement from the Ontario Government for all hours spent directly caring for clients.
Key Responsibilities
- Observe and evaluate the client’s or family’s home, personal situation and care plan to determine if changes are needed; advise supervisor accordingly.
- Contribute to the development of the personal support care plan under the direction of the Personal Support Program Manager; document the plan with information from the funder, Manager, and client/family.
- Implement changes to the care plan under the Manager’s guidance for safety, liability, and financial reasons.
- Report changes in the client’s or family’s needs to the supervisor or Manager for adjustments to the care plan.
- Perform personal support tasks as needed, including light cleaning, meal preparation, clothing care, shopping, and maintaining household safety.
- Provide personal care as directed, including assistance with walking, stairs, bed mobility, eating, dressing, and personal hygiene.
- Provide simple bedside care under the direction of a nurse and offer respite care to clients as required.
- Support clients’ families by providing child care as needed to allow for family relief.
- Organize daily activities and set priorities for personal support and other tasks under the direction of the Manager or supervisor.
- Complete daily call checks and other forms to maintain operation of the personal and home support service.
- Assess client communication patterns and ability to relate.
- Communicate with health team members regarding client or family needs and inform a supervisor of any changes or concerns.
- Work in compliance with Occupational Health and Safety legislation, regulations, and VON policies; follow risk management guidelines to promote client safety.
- Perform other duties as assigned.

External and Internal Relationships
- Engage with the immediate supervisor or Personal Support Program Manager regarding changes to client plans and related information.
Education, Designations and Experience
- High school diploma (secondary school education preferred).
- Certification in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide.
- Current Standard First Aid Certificate or CPR Level C, if applicable to the work performed.
- Some household management experience and experience with elderly or ill individuals or children.

Other
- Must have access to a personal vehicle and possess a valid driver’s license and proof of vehicle insurance.
- Current satisfactory Criminal Records Check; Vulnerable Sector Search and/or Child Abuse Registry Check may be required.
- Available to work flexible hours; must be able to wear PPE.
Working conditions and physical demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying with proper technique, bending, reaching, kneeling, walking, sitting, standing, and climbing stairs.
