Are you a caring and compassionate Personal Support Worker (PSW) looking to make a real difference? Join our CBI Home Health’s Guelph-Wellington OHT Leading Practice team, where we’re doing things in a new and exciting way! As a PSW, you’ll work closely with a local primary care team to provide hands-on, person-centered care that helps people stay healthy and independent in their own homes. You’ll be part of a supportive home health team focused on preventing health problems before they happen, helping patients avoid trips to the hospital and live happier lives.

What you will need to be successful

• Your Personal Support Worker certificate or equivalent:
  • Valid Certificate as Home Support Worker, I, II or III, Health Care Aid, Developmental Support Worker; Or
  • Students currently enrolled (post first year) in health care programs such as Nursing (RN or RPN), Rehab, Para Medicine; Or
  • A valid certificate as an RN, RPN, Practical Nurse, Midwife, or Physician (domestic or internationally trained)
• Current and valid Standard First Aid and CPR (Level C) Certification
• A clear Criminal Record Check with Vulnerable Sector Screening within the last 12 months
• Proof of a negative TB test within the last 12 months and an up-to-date immunization record
• Ability to travel from home to home within the community- a valid driver's license, vehicle and car insurance is required for this role
  • Travel required within East Wellington rural communities such as Rockwood, Mapleton, Drayton & Mount Forest
• Commitment to work every other weekend
• Strong verbal and written English communication skills required; the ability to communicate in French or other languages is considered an asset
• Reliability and strong organization skills
• Ability to safely lift up to 35 lbs. (16 kg)
