Personal Support Worker (PSW, DSW, HSW, HCA) (RPT 0.5)

Job Summary:

The Personal Support Worker provides personal support and homemaking services to clients and families within the framework of VON policies, procedures, and standards. The PSW collaborates with members of the healthcare team to meet clients' needs, aiming to promote independence and well-being.

This position is eligible for a $3.00 per hour wage enhancement from the Ontario Government for all hours spent directly caring for clients.

Key Responsibilities:

  • Observe and evaluate clients' or families' home and personal situations, and care plans to determine if adjustments are needed, reporting to the supervisor accordingly.
  • Contribute to developing the personal support care plan under the direction of the Program Manager, incorporating information from funders, the manager, and the client or family.
  • Implement changes to the care plan for safety, liability, and financial reasons, under the guidance of the Manager.
  • Report any changes in the client's or family's needs or home situation that require adjustments to the care plan.
  • Perform personal support tasks based on client and family needs, including light cleaning, meal preparation, clothing care, shopping, and household safety monitoring.
  • Provide personal care as directed, including assistance with walking, stairs, bed transfers, eating, dressing, and hygiene.
  • Assist with bedside care under a nurse's direction when required.
  • Provide respite care as needed.
  • Support families by providing child care to facilitate family relief.
  • Organize daily activities, setting priorities under the supervision of the Program Manager or immediate supervisor.
  • Complete daily call checks and other documentation to ensure efficient service operation.
  • Assess clients' communication and relational abilities.
  • Communicate with the healthcare team regarding homemaking, physical, and emotional needs, reporting concerns or changes.
  • Comply with Occupational Health and Safety legislation, regulations, and VON policies.
  • Follow risk management policies to promote client safety.
  • Perform other duties as assigned.

External and Internal Relationships:

  • Maintain regular communication with the supervisor or Program Manager regarding client plans and relevant information.

Education, Designations, and Experience:

  • High school diploma.
  • Certificate in Home Support Worker, Personal Support Worker, Developmental Services Worker, or Health Care Aide.
  • Nursing students who have completed their first year of Practical Nursing or BScN are eligible to apply.
  • Internationally Educated Nurses (IENs) are encouraged to apply.
  • Experience in household management and working with elderly, ill individuals, or children.
  • Current Standard First Aid Certificate.
  • Flexible working hours.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work independently and problem-solve.
  • Adaptability to change and various social and economic environments.
  • Must have a personal vehicle, valid driver's license, and proof of vehicle insurance.
  • Satisfactory Criminal Records Check required at start of employment.
  • Vulnerable Sector Search and/or Child Abuse Registry Check may be required.
  • Ability to speak French is an asset in designated areas.
  • Ability to wear Personal Protective Equipment (PPE).
