Job Summary:

The Personal/Home Support Worker provides personal support and homemaking services to clients and families within the framework of VON policies, procedures, and standards. The PSW/HSW collaborates with members of the health care team in meeting the needs of clients to attain an optimal level of independence and wellbeing.

Key Responsibilities:
  1. Observes and evaluates the client’s or family's home, personal situation, and care plan to determine if changes are needed.
  2. Contributes to the development of the personal support care plan under the direction of the Personal Support Program Manager.
  3. Implements changes to the care plan under the guidance of the Manager.
  4. Reports changes in the client’s or family's personal support needs to the supervisor.
  5. Performs personal support tasks including light cleaning, meal preparation, and monitoring household safety.
  6. Provides personal care as directed, including assistance with mobility and personal hygiene.
  7. Provides simple bedside care under the direction of a nurse.
  8. Provides respite care to clients as required.
  9. Supports clients’ families by providing childcare as needed.
  10. Organizes daily activities and priorities under the direction of the Manager.
  11. Completes daily call checks and forms as required.
  12. Assesses the client's communication patterns.
  13. Communicates with health team members regarding client needs.
  14. Functions in compliance with Occupational Health and Safety legislation and VON policies.
  15. Other duties as assigned.

Education, Designations and Experience:
  1. High school diploma.
  2. Secondary school education is preferred.
  3. CCA Certification required in Nova Scotia.
  4. Certificate in Home Support Worker, Personal Support Worker program, or Health Care Aide.
  5. Current Standard First Aid Certificate or CPR level C Certificate, if applicable.
  6. Experience working with elderly, ill individuals, or children.

Skill Requirements:
  1. Strong communication skills, both verbal and written.
  2. Effective communication with clients and families.
  3. Good assessment skills.
  4. Awareness of community resources.
  5. Ability to work effectively in a multidisciplinary team or independently.
  6. Ability to take direction well.
  7. Ability to adjust to different social and economic environments.
  8. Capacity to adhere to strict confidentiality.

Other Requirements:
  1. Access to a personal vehicle for work and possession of a current driver’s license and proof of insurance.
  2. Current and original satisfactory Criminal Records Check.
  3. A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required.
  4. Available to work flexible hours.
  5. Must be able to wear Personal Protective Equipment (PPE).
