A leading healthcare facility in Toronto is seeking a Personal Support Worker for its Complex Continuing Care Unit. The candidate will support patients with personal care needs, maintain relationships with clients and families, and enhance patient dignity and well-being. Position Type: Permanent Part-Time

Shift Type: Day, alternate weekends (subject to change)

Hourly Pay Rate: $26.37 - $27.05
Bi-Weekly Hours: 30 Hours

Hours of Work: 7.5hrs /shift

The Complex Continuing Care Unit provides care for older patients with complex medical conditions in a non-acute hospital setting. The Complex Continuing Care Program helps patients and their families identify strategies that support quality-of-life goals as well as short term and long term care needs.

Responsibilities include but are not limited to:

  • Supports, in consultation with the Registered Practical Nurse/Registered Nurse, the physical and personal care needs of the client including, but not limited to: hygiene, grooming and dressing, toileting, recording nutrition and hydration intake, provision of preventative skin care
  • Supports patients’ independence, dignity, social, emotional and cultural well - being
  • Initiates and maintains a relationship with the client, their family and other care providers

Qualifications include but are not limited to:

  • Current P.S.W. certificate from a recognized community college or educational institute or equivalent combination of education and experience
  • Experience in a Post-Acute or Acute Care setting preferred
  • Training in Gentle Persuasive Approach and U-First preferred
  • Psychogeriatric experience, i.e. mental health assessment and intervention and working with responsive behaviours including various types of dementia, delirium, mental health issues preferred
  • Ability to work independently and as a part of the interdisciplinary team, and effectively manage challenging situations
  • Experience working with clients with cognitive impairment/dementia and experience working with clients who exhibit high intensity responsive behaviours preferred
  • Excellent interpersonal, communication and problem solving skills
  • Basic computer skills – Meditech, Microsoft Word and Excel preferred
