Job Description: The PSW/HCA/CCA provides direct personal support and care to patients, fulfilling their physical, intellectual, emotional, social, and spiritual needs in accordance with policies, procedures, and work routines. Assists in providing direct care such as ambulation, feeding, bathing, and grooming.

Qualifications:

• Completed a PSW Program or HCA/CCA certification that meets the appropriate standards for the position.
• Experience in long-term care or community home care, with exposure to dementia and palliative care preferred.
• Knowledge of care planning systems and documentation.
• Ability to communicate positively with staff, residents/patients, and families.
• Ability to provide care following legislation, routines, and care plans; proficient in English reading, writing, and verbal communication.
• Experience working independently in community home care is an asset.
• Flexibility to work on different units as per facility requirements.
