Requisition Details: Employment Status: Permanent, Part-time (0.4 FTE). Program Name: Assisted Living. Number of Hours Bi-Weekly: 30. Work Schedule: Days, Evenings, Weekends. On Call: Yes. Existing Vacancy: Yes – we’re currently hiring candidates for an existing vacancy in this position.

Job Summary: Personal Support Workers will provide companionship, personal care, and health maintenance services to clients in their homes or community, aligning with VON’s philosophy, policies, standards, and legislation. You will collaborate with the Healthcare and Personal Support teams to support clients in achieving independence and well-being.

This position is eligible for a $3.00 per hour wage enhancement from the Ontario Government for all hours spent directly caring for clients.

Key Responsibilities:

  • Assess and evaluate clients’ or families’ situations and care plans, suggesting changes as needed.
  • Assist in developing and implementing personalized care plans under supervision.
  • Report any changes in client needs to the supervisor.
  • Perform personal support tasks including light cleaning, meal prep, clothes care, shopping, and household safety monitoring.
  • Provide personal care, such as assistance with mobility, hygiene, and daily activities.
  • Support clients with bedside care and respite care as needed.
  • Coordinate daily activities and prioritize tasks.
  • Complete required documentation and calls to ensure service efficiency.
  • Communicate effectively with clients, families, and health team members.
  • Adhere to safety, confidentiality, and risk management policies.
  • Perform other duties as assigned.

External and Internal Relationships: Regular communication with supervisors and team members regarding client care and needs.

Education, Designations, and Experience:

  • High school diploma; secondary education preferred.
  • CCA Certification in Nova Scotia or equivalent Home Support Worker/Health Care Aide certification.
  • Current Standard First Aid or CPR Level C certification.
  • Experience with household management and caring for elderly, ill, or children.

Skill Requirements:

  • Effective verbal and written communication skills.
  • Ability to assess and relate well to clients and families.
  • Knowledge of community resources.
  • Ability to work independently and in teams.
  • Respect for confidentiality and adaptability to various environments.

Other Requirements:

  • Own transportation, valid driver’s license, and proof of insurance.
  • Criminal Records Check and possibly Vulnerable Sector Search or Child Abuse Registry Check.
  • Flexibility in working hours and use of PPE.

Work conditions involve physical activity, including lifting, bending, reaching, walking, sitting, standing, and stair navigation.
