Job Summary:
The Personal Support Worker provides personal support and homemaking services to clients and families within the framework of VON policies, procedures, and standards. The PSW collaborates with members of the healthcare team in meeting the needs of clients to attain an optimal level of independence and well-being.
This position is eligible for the $3.00 per hour wage enhancement from the Ontario Government for all hours spent directly caring for clients.
Key Responsibilities:
- Observes and evaluates the client’s or family's home, personal situation and care plan to determine if changes are needed and informs the supervisor accordingly.
- Contributes to the development of the care plan under the direction of the Program Manager, with information from the funder, the Manager, and the client or family.
- Implements changes to the care plan for safety, liability, and financial reasons under the guidance of the Manager.
- Reports changes in client or family needs to the supervisor or Program Manager.
- Performs personal support tasks such as light cleaning, meal prep, clothing care, shopping, and household safety monitoring.
- Provides personal care including assistance with walking, stairs, bed transfers, eating, dressing, and hygiene.
- Provides bedside care under nurse supervision as needed.
- Offers respite care to clients and supports families with child care as required.
- Organizes daily activities and sets priorities under supervision.
- Completes daily call checks and other forms to ensure effective service operation.
- Assesses communication patterns and ability to relate of clients.
- Communicates with the health team about client needs and reports concerns or changes.
- Complies with Occupational Health and Safety laws, regulations, and VON policies to promote safety.
- Adheres to risk management policies to ensure client safety.
- Performs other duties as assigned.
External and Internal Relationships:
- Engages with supervisors and Program Managers for updates and changes to client plans.
Education, Designations and Experience:
- High school diploma; secondary education preferred.
- CCA Certification in Nova Scotia.
- Certificate in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide.
- Current Standard First Aid or CPR Level C Certificate, if applicable.
- Experience with household management and caring for elderly, ill individuals, or children.

- Access to a personal vehicle with valid license and insurance.

Working Conditions and Physical Demands:
This role involves physical activity like lifting, bending, reaching, and climbing stairs, requiring good body mechanics. Tasks include walking, sitting, standing, and fine hand movements throughout the day.
