Baycrest Hospital is seeking a dedicated Personal Support Worker to join the Complex Continuing Care Unit (5 West) on a temporary part-time basis. You will collaborate with nursing staff to provide essential personal care, monitor hydration and nutrition, and support daily living activities for medically complex older adults in a non-acute setting.

Qualifikationen

• Current P.S.W. certificate from a recognized community college or educational institute or equivalent combination of education and experience
• Experience in a Post-Acute or Acute Care setting preferred
• Training in Gentle Persuasive Approach and U-First preferred
• Psychogeriatric experience, i.e. mental health assessment and intervention and working with responsive behaviours including various types of dementia, delirium, mental health issues preferred
• Ability to work independently and as a part of the interdisciplinary team, and effectively manage challenging situations
• Experience working with clients with cognitive impairment/dementia and experience working with clients who exhibit high intensity responsive behaviours preferred
• Excellent interpersonal, communication and problem solving skills
• Basic computer skills – Meditech, Microsoft Word and Excel preferred

Job Summary

The Complex Continuing Care Unit provides care for older patients with complex medical conditions in a non-acute hospital setting. The Complex Continuing Care Program helps patients and their families identify strategies that support quality-of-life goals as well as short term and long term care needs.
