Qualifications

Responsibilities

  • Complete documentation according to Carefor policies.
  • Report any hazards or violations of health and safety regulations.
  • Maintain current certifications: Personal Support Worker, First Aid, CPR.
  • Participate in a defined pension plan and access professional development opportunities.
  • Ensure workplace incidents are reported in a timely manner.
Job description
Overview

The Personal Support Worker (PSW) provides personal support and homemaking services under the supervision of the Care Team Supervisor within Carefor Health and Community Services and the Pembroke Civic Complex policies, standards and quality & risk management. The PSW collaborates with the health care team to meet residents’ needs to attain an optimal level of independence and wellbeing. The PSW works within their scope of practice and informs the manager or designate if additional training is required.

Responsibilities
  • Assist or provide care according to the Resident Care Plan, including personal care, lifts and transfers, observing nutritional status, light housekeeping, and assisting with prescribed medications as required.
  • Observe residents for changes in behaviour or signs or symptoms of decreasing health status and report to the Care Team.
  • Chart care given and provide verbal/written reports to the RPN or other team members on any unusual behaviour or observations.
  • Evaluate the individual’s need for changes in the personal support care plan and make recommendations to the RPN or designate.
  • Contribute to the development of the personal support care plan by reporting observations from the resident and/or family regarding needs and the effect of the care provided.
  • Plan and organize each day’s activities, setting priorities for personal support and other activities to effectively utilize time, energy, supplies and equipment.
  • Complete documentation/paperwork according to Carefor policies and procedures.
  • Review all forms of documentation and communication to remain current on residence operations and resident care (care notes, emails, memos, communication book, and verbal updates).
  • Function and report in compliance with Occupational Health and Safety legislation, regulations and Carefor policies and procedures.
  • Report the absence of or defect in any equipment or protective device and report hazards or violations of Health and Safety Act or regulations.
  • Ensure all workplace incidents are reported in a timely manner in accordance with legislation and Carefor policies and procedures for workplace injuries and incidents.
  • Maintain current certifications: Personal Support Worker or Health Care Aide, Standard First Aid, and CPR Level C.
