Qualifications

As a member of an inter-professional team reporting to the Care Manager, the role of the Personal Support Worker (PSW) is to ensure the provision of excellent care and support to the patients of hospice house, their families, and caregivers, in accordance with hospice house and organizational policies and procedures. The PSW is responsible for providing routine care for all activities of daily living, supporting patients and families, and assisting volunteers as needed. The PSW helps maintain a welcoming, home-like environment and contributes to the daily operations of the program. They support the efficient operation of the house through continuous quality improvement, aligning their duties with the mission, vision, and values of the Providence Care Centre.

Skills & Qualifications Required
  • Diploma or certificate from an accredited PSW program, private career college, or Adult and Continuing Education School Board Program, as per the Ontario Qualifications Framework.
  • Palliative Care Certificate such as LEAP or PACE, or willingness to complete within 6 months of hire.
  • Valid CPR/First Aid Certification.
  • Minimum of 2 years' experience in hospice palliative care in community, hospice, hospital, or long-term care settings.
  • Experience in pain or symptom assessment and management.
  • Experience working with an interdisciplinary team and supporting volunteers.
Position Responsibilities
  • Provide direct care to patients according to their care plan and wishes.
  • Assist with bathing, oral, nail, and hair care as per the care plan and patient preferences.
  • Apply topical medications and protective dressings following pharmacy instructions.
  • Assist with dressing patients daily, respecting their preferences.
  • Provide bedtime care in accordance with the care plan and patient preferences.
  • Assist patients with bathroom needs, including use of commode, bedpan, or urinal.
  • Perform incontinence, colostomy, or catheter care as needed.
  • Assist patients with mobility as per the care plan.
