SUMMARY
The Sinai Hospital to Home Program assists clients transition safely from hospital stay to continue care for a maximum of 16 weeks in their homes. As a PSW with Circle of Care, you will provide the highest quality of care to your clients in the community. You understand that your clients are dealing with complex illness or frailty, and simple tasks can prove challenging for them. Your job involves providing personal care to seniors living in their homes or other congregate living settings including grooming, bathing, dressing, toileting, safe transfers and meal preparation. You will be asked to perform housekeeping, and companionship or emotional support. You are comfortable travelling to and from clients' homes throughout the day within the community to service a highly vulnerable population with the care they need.
